SMS Terms & Opt-In
Effective Date: April 27, 2026This page describes Mindfulous's SMS (text message) program: how patients and prospective patients opt in, the kinds of messages we send, how often, how to stop, and your privacy rights. This page exists to comply with US carrier (CTIA / A2P 10DLC) requirements and to be transparent about our communication practices.
Program Description
Mindfulous Integrative Health Clinic ("Mindfulous", "we", "us") sends SMS messages from +1 (415) 915-8997 for the following purposes:
- Intake form delivery — when a caller asks to receive our patient intake form by text
- Appointment-related transactional messages — booking link delivery, appointment confirmations and reminders, rescheduling notices, intake completion reminders
- Care-coordination follow-ups — messages from a member of the practice team in response to an inquiry
Mindfulous SMS is not used for marketing or promotional messages. We do not sell, rent, or share your phone number with third parties for marketing purposes.
How You Opt In
You can opt in to receive SMS messages from Mindfulous in either of two ways:
- Verbal opt-in during a phone call. When you call our practice line at +1 (415) 375-0892, our AI voice concierge ("Mindy") may ask you the following:
"I'd love to text you our intake form link — would that work? I just need your cell number."
If you say yes and provide a cell number, we treat that as your express consent to receive a transactional SMS at the number you gave us. Each call is recorded and transcribed; the recording and transcript serve as the durable record of your consent and are retained for at least 24 months for compliance purposes. - Written opt-in via the intake form. Our online intake form at https://docs.google.com/forms/d/e/1FAIpQLSc5BWUOFpMiJm4lmAvIBWrdJDhkKnOVm7n_-Wv7Uz0ELeh-WQ/viewform includes a phone-number field. By submitting it you consent to receive transactional SMS related to your intake and appointments.
You can revoke consent at any time (see "How to Stop" below). Consent to receive SMS is not a condition of receiving care from Mindfulous.
Sample Messages
The first SMS you receive from us identifies Mindfulous and includes opt-out instructions. Examples:

Mindfulous: Reminder — your appointment with Dr. Chaabo is tomorrow at 2:00 PM PT. Telehealth link: [link]. Reply STOP to opt out.
Mindfulous: Brandi here — quick check-in, please complete your enrollment forms in your patient portal before your visit. Need a hand? Reply HELP. Reply STOP to opt out.
Message Frequency
You will receive up to 5 messages per inquiry or appointment cycle, on average. Frequency depends on what you need from us — a single intake-link request typically results in 1 message, while a full new-patient onboarding may include 3-5 messages over the course of 1-2 weeks (welcome, scheduling, reminders).
Message and Data Rates
Message and data rates may apply, per your wireless plan. Mindfulous does not charge for SMS messages — your carrier may.
How to Stop Messages
You can opt out of all SMS messages from Mindfulous at any time:
- Reply
STOP,END,CANCEL,UNSUBSCRIBE, orQUITto any message we send. We will send a single confirmation reply and no further messages. - Email hello@mindfulous.com with the subject line "SMS opt-out" and the phone number to remove.
- Call (415) 375-0892 and ask any team member or our voice concierge to remove your number from SMS.
Once you opt out, we will not send you SMS messages again unless you explicitly opt in a second time.
How to Get Help
Reply HELP to any SMS we send, and you will receive an automatic response with our contact information. You can also email hello@mindfulous.com or call (415) 375-0892.
Carriers and Compatibility
Mindfulous SMS is supported by all major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages. Wireless carriers supported include AT&T, T-Mobile, Verizon, U.S. Cellular, and others.
Privacy and HIPAA
Mindfulous treats SMS as a non-HIPAA-secure channel and uses it only for non-PHI communications: intake links, appointment reminders, scheduling logistics, and our return-call coordination. We do not include diagnoses, medication names, lab results, insurance member IDs, or other Protected Health Information in SMS. For clinical messaging, established patients use the secure Charm patient portal.

Changes to This Policy
We may update this SMS Terms page from time to time. The "Effective Date" at the top of this page reflects the most recent revision. Material changes will be highlighted at the top of the page for at least 30 days.
